Each new potential client must complete intake documents in full before being able to schedule a first appointment.
How much will my copayment be?
Your copay will be verified after intake documents are completed. Please allow up to 48 hours for verification process.
I don’t have my insurance card with me, can I still receive treatment?
Your insurance information can be verified using your name, date of birth, and full address.
